Dear Simon,
I am sorry it has not been possible for me to meet you in person. You did not attend
appointments with me at Silver Street, nor did you allow me and your Care Coordinator
Soobah to enter your property when we recently came to visit you.

Soobah and I would like to see you with a view to ascertain whether there is anything that our
team could offer to support you. I therefore encourage you to make contact with us at the
number above to book such appointment at you earliest convenience. Should you chose not to
do so, we will assume you don’t require our support and we will consider a discharge from our
team back to the care of your GP.
In the meantime, should you feel you need it, please contact the Duty staff on the above
telephone number between 9am and 5pm and the CRHTT on 0208 702 3800 between 5pm
and 9am if there is a crisis.
