Mental state: Simon appeared settle and stable in his mental state presentation last night. He
was with his parent visiting him at the time of taken over the shift. He was happy at their
visit. Observed interacting with co-patients appropriately and appeared to have slept through
the night.
Nutrition: No concern with food and fluid intake during the shift.
Medication: He continue to refuse anti-psychotic medication and takes only 1mg
Lorazepam.
