carried out for this patient within the last
twelve months with regards to:
9 Cardiovascular Disease
10 Chronic Obstructive Pulmonary
Disease
11 Diabetes
12 Height & Weight
13 Blood test results
It would be very helpful if you could also provide
details of any other significant physical health
conditions the client has been diagnosed with or is
being investigated.
We will update you routinely and following
reviews and I look forward to working with you to
support Mr Simon CORDELL with his mental
health problems.
