Doctors report!
No visit still never felt well tablets messed up my sleep pattern too tired to do anything,
Dizzy, feeling sick, tangential thinking, irritable.

them reports, this includes a personal statement and a copy of the assessments transcripts that

Simon you are meant to be trying to get your records corrected you want them to have as
little as possible on file about you why are you doing this in such a way, they can hold more
on you.
