Getting wrongly detained while Working on my report in the;
Chase Farm Mental Hospital on an Assessment Wing!
A nurse called Simon who was on night shift came to the room that I got detained in he tried
to give me medication that was not prescribed by a doctor
The medication that he tried to give me I recognised from St Ann’s Hospital
The name of the medication is Olispine
I told him that I understood that no doctor had been on duty to order this a s Dr Greenside
was away on leave.
I noticed that the TV in the main room has broken as it has gotten old this did get reported
to the nurses on the Dorset ward, what I noticed had happened is that the back light that
glows up the picture on the screen had failed and could not get fixed, this meant that no
person could watch the Tv.

Mental State: Spent most of his time in the room. Spoke to staff in depth about his beliefs
about his neighbours and previous admission in hospitals and police investigations. He
stated that he beliefs that they are out to get him and that there is a conspiracy theory that
they are all out to get him. He has set up a website with details, videos and recordings.
Social Inclusion: Some interaction with peers in the lounge area.
Investigation: Nil on the shift
Physical Health: Nil complained or observed on the shift. Observed eating and drinking
adequately.
Medication: Took medication as prescribed.
Personal Care and Activities: Observed to have good personal hygiene
Risk and observation Level: Nil observed to self or others on the shift. He is nursed on
general observations.
