MENTAL STATE He Remains fairly calm and keeping to self on the ward, though still lack
insight into his mental state.
SOCIAL INCLUSION Interacting well with peers and staff.
MEDICATION He is not on any medication at the moment
PHYSICAL HEALTH Nil reported on the shift. He is eating and drinking well
INVESTIGATION Nil carried out on the shift.
PERSONAL CARE 6 ACTIVITIES Appears satisfactory. He did not participate in any ward
base activity, but was observed watching tv in his bedroom

Getting wrongly detained while Working on my report in the;
Chase Farm Mental Hospital on an Assessment Wing!
WHEN I GOT UP OUT OF MY BED, I TOOK MY SHOWER IN MY ROOM THEN I WENT OUT AND
GOT A CUP OF TEA
I Ordered Pizza for Everyone.
I could hear shouting in the main room and went to see if everyone was ok
I found a lady patient crying who I get along with since I arrived on the wing
When asking her why this was happening, she told me that
21:00 Hours I had a problem on the wing with a new addition
I noticed that the man came onto the Dorset wing last night as he was playing chess with
one of the staffs in the main room.
The Tv got fixed: Changed for a new one.”
