I have today spoken to the Mother of the above as I was unable to make telephone contact
with the patient. I informed Mrs Cordell that two male members of staff, one a Doctor and
the other a nurse would be making a home visit to her Son on Friday 31st August 2018 at
9.15am to carry out an Assessment. Mrs Cordell informed me that she would not be present
at the appointment as she herself has a hospital appointment which she had been waiting
for a while to ascertain and could not cancel this. I informed her that the Doctor and the
Nurse would still attend for the appointment at the client’s home address and she informed
me that she would let her Son know. Mrs Cordell said that her Son may not be happy about
the visit but none the less she will inform him.

During the consultation I will add that: ‘we have reported this to the information
commissioner, and are liaising with them. Any concerns about the information should be
shared back to Rachel Yona and the patient and his mother should contact her with any
questions.
