Who you are and in what capacity you know
the patient, how long you have worked with
them: My name is Soobah Appadoo? I am a CPN
with the above-named team. I have been asked to
compile this social circumstance report in support
of above-named patient’s Mental Health Review
Tribunal (MHRT) appeal against his detention
under Section 2 of the Mental Health Act. Mr
Cordell was admitted on section 2 on the 26th
October 2018.
Mr Cordell was previously under the care of the
Early Intervention Team for 3 years. The Early
Intervention Team discharged him in June 2018
and at that point he was referred to our team.
There is a suggestion on RIO notes that he did not
engage well with that team.
