I have made several attempts to call you but have not been successful. If you do wish to continue with your
complaint, and therefore wish for your data to be transferred please respond to this email by copy and pasting
the following sentence and inserting your name in the space:
“I (YOUR NAME) consent to information regarding referral and data held by VoiceAbility being transferred to
POhWER by the 1st April 2017”
