Mental State Presented as polite, calm and stable in mental state. He was observed to
be keeping to self in his bed space mostly watching tv.
Social Inclusion Selectively interacts with other peers and staff. His mother visited
Investigation Nil on the shift
Physical Health Nil reported or observed on the shift. He had hot drink and snack and
appeared to have slept well Medication: Simon is not on any night medication
Personal Care and Activities Appears satisfactory and was observed watching television.
Risk and observation Leve Nil observed to self or others on the shift. He is nursed on
general observations.

Getting wrongly detained while Working on my report in the;
Chase Farm Mental Hospital on an Assessment Wing!
At 11:30 Hours I spoke to a lady called Dr Bruise and I asked her if I could speak to a Dr
Green side about getting some trust to be able to leaver the ward, when doing so she
agreed to look into the issue for myself.
Soon afterwards I was alone in room 24 on the Dorset Ward when Dr Greenside knocked on
the door, I then on answered the door to get invited into a meeting
There were two other doctor presents in the room alongside with Dr greenside himself.
The conversation stayed polite and reasonable through
When taking an understanding to what was getting explained to me, I took the
understanding that the doctors still wanted to further assess me, due to having a
professional belief that I may have some form of mental health issue to which I know that I
did explained
