MENTAL STATE 1:1 with Simon who was polite and engaged well. He became anxious
                   when he started discussing the tribunal. He said he was wrongly accused by police and
                   does not believe he is ill. He still believes his neighbours are wrong and he is not taking
                   any responsibility for any harm towards his neighbours. He thanked staff for their help
                   and support on the ward. He agreed to have a copy of his care plan and discussed it at
                   length.
                   SOCIAL INCLUSION Mostly on computer in his room during the shift, showing staff his
                   writing about the tribunal.
                   INVESTIGATION None carried out.
                   PHYSICAL HEALTH He did not report any problem.
                   He ate and drank well.
                   MEDICATION Not on regular medication.
                   PERSONAL CARE & ACTIVITIES Is satisfactory, he interacted with fellow patients.
                   RISK AND OBSERVATION LEVEL Of slow progress in his mental state due to non-
                   compliance.

               Getting wrongly detained while Working on my report in the;
               Chase Farm Mental Hospital on an Assessment Wing!
               Monday Morning, I awoke to the now normal daily routine and soon after spoke to a kind
               gentleman who is a part of the estate management team, he oversees my problem of cause
               and retried it with ease E.G “broken bathroom window due to not being able to shut
               correctly”
               What doctor green explained to me; next I think I took very well. He explained to me that he
               wanted to put me onto a section three of the Mental Health Act 1983.
               How it happened!
               Just before lunch time Doctor Greenside came to my room and asked me for a meeting with
               me. I agreed to what he had asked and walked with him into another room, when sitting
               down together we were both left quietly alone and then Doctor Greenside started to
               explain to me that he wanted to place me of the section 2 of the Mental Health Act 1983
               and on to A section 3 of that act.
               I took my time and asked him why he believed this was warranted as to the initial reasons to
               why I had been placed on a section two had been proven to be inaccurate to the assessment
               documentation of its statement of facts.
               We went through what I had gotten accused of when first getting detained to hospital,
               which is: --
               1. Mr. Cordell is known to psychiatric services but has not engaged with them in recent
               years.

               This information is undermining towards the defamation of the definition of my character
               and influences a person beliefs into believing that I have gotten treated under the Mental
               Health Act 1983 as a person who has been diagnosed with a mental illness, to which I have
